  • Articles of Incorporation

  • DBA Name Documentation (If Applicable)

  • Profit and Loss Statement

  • Most recent three months of bank statements

  • Cyber Liability Insurance

  • Certificate of Business Insurance

  • Proof of payment processing and good standing with recently closed bank

  • Social Security Number (SSN) or Passport Number

  • US Government Issued Photo ID or Passport Data
